On the menu below please find several important areas to familiarize yourself with before your first appointment as well as forms to be printed and filled out and brought to your first appointment. Your therapist will direct you to which forms are the appropriate ones to complete.
Each year we ask our patients to review and sign our HIPPA/Privacy Form as well as a financial policy form.
